Advance Care Planning Forms
FOR MASSACHUSETTS RESIDENTS:
- Health Care Proxy Form - Complete this form to designate your health care agent - the person you would like to make health care decisions for you if you were to become unable to make them for yourself. You'll find information on how to complete this form in the booklet, A Guide for a Better Ending.
- Personal Wishes Statement - Use this form to communicate your specific wishes to your health care agent to guide him or her in making decisions on your behalf.
