You can contact the UniCare State Indemnity Plan by email, in writing, or by telephone for the following information about your medical benefits:

  • general information
  • notification of admissions
  • notification of outpatient procedures
  • preferred vendor information

    Contact the Plan via e-mail. We will respond to your inquiry within three business days. If we need additional information to answer your question, we may need to contact you by telephone or by mail to obtain this clarification.
