Welcome to the UniCare State Indemnity Plan's web page for GIC Coordinators. Request Plan Materials - Use our online form to order member materials, including enrollment materials such as the PLUS Provider Directory and our Plan Option Comparison Guide.

Forms and Documents - View or print various Plan documents and forms available online, including Member Handbooks.

Avoiding Balance Billing with Non-Massachusetts Providers - This information applies to non-Medicare members or their eligible dependents. These members can avoid balance billing when they are traveling outside Massachusetts by using our out-of-state providers. In addition, all Basic members, and PLUS members who reside in Connecticut, Maine, New Hampshire or Rhode Island, also have access to UniCare network providers in their state.

Notification Requirements - Find out which procedures require pre-notification and how much notice is required.

Provider Search - Search our online directory for the names of PLUS providers. PLUS Plan members who use PLUS providers receive a higher level of benefits than they do if they use providers who are not in the PLUS network. PLUS members who live in Connecticut, Maine, New Hampshire and Rhode Island also have access to additional network providers in their state at the same benefit level as with PLUS providers. Click on "out-of-state providers" on the provider search page to check these provider listings. When prompted to enter the plan name, select: UniCare PPO – PHCS.
