Search for a UniCare Out-of-State Provider

If you have received a UniCare card from the Plan, or if you are traveling outside your home state and need urgent care, here’s how to locate a UniCare out-of-state provider that will not balance bill you for covered services. Please be sure to select the correct network.
Important: If you need mental health/substance abuse treatment or the Enrollee Assistance Program (EAP), you must contact United Behavioral Health (UBH), the administrator for these services. You will be subject to balance billing if you use a mental health/substance abuse provider that is not in the UBH network.

Please follow the instructions below carefully to ensure that you access the correct network of providers.
- Select “Member Personalized Search” and when asked to enter your member number, enter: GIC, as shown below. (You can use upper or lower case letters.)

- Select the plan that applies to you:
- If you have received a UniCare card (card with green wave) from the Plan:
Select the name of the out-of-state plan network that appears on your UniCare card.
- If you are traveling outside your home state:
Select “Travel Access.”
- Enter the criteria for your search and a list of providers that meet your criteria will be displayed.
* To view the Provider Finder information in Spanish, first login by using the Member Personalized Search
function (see login information above). Then click “Espanol” at the top right of the Provider Finder page.
